Puppy Reservation Form

There is a $50.00 fee to reserve an unborn puppy. An additional $100.00 payment will be required when you are notified by email that a puppy meeting your requirements has been born. |
To reserve a puppy, fill in the spaces below with the requested information, and be sure to include your email address. If you make a mistake, you can press the Reset Button at the bottom of the page to clear the form and start over. Then, when you are finished, you can print out the completed reservation form and send it along with your payment to the address below.
If you would prefer, you can also send your completed reservation form to us by email (we will receive it a lot faster). After you have filled out the form, just press the Submit button at the bottom of the page, and your form will be immediately sent to us by email. Then just send us your payment by regular mail. Email reservations will be held for 10 days until your check arrives. Your reservation for your puppy is not secured until your check is received.
Please make all checks payable, in US dollars, to Castlebar Elkhounds. All deposits are applied toward the purchase price of the puppy.
All deposits are non-refundable.
Puppies are shipped in the continental USA only. Please call or email to see if the puppy can be shipped to your state. Shipping costs are in addition to the cost of the puppy. |
